     Step 1:  Download and Complete the Athletic Physical/Medical Packet
    • Download the Season Specific Sports Physical/Medical Packet below.
     
    • The Pages are designed for Double-Sided Printing but can be printed single-sided.
     
    • Packets are available in the North & South offices if you are unable to print on your own.

    FALL: Fall
     
    All physicals must be within 365 days of August 10, 2017 ( first official day of Fall practice).
     
    All the physical forms should be turned,  the FamilyID.com Online Registration completed & the Activity Fee's satisfied at least 7 DAYS PRIOR to insure your child's paperwork being cleared for the 1st day of practice/tryouts.
     


    WINTER--PHYSICAL/MEDICAL PACKET--DOWNLOAD HERE
     
    WINTER: Fall
     
    All physicals must be within 365 days of November 6, 2017 (Swimming), November 15, 2017 (Bowling) and November 20, 2017 (Basketball, Cheerleading, Indoor Track & Wrestling) which are first official days of Winter practices.
     
    All the physical forms should be turned,  the FamilyID.com Online Registration completed & the Activity Fee's satisfied at least 7 DAYS PRIOR to insure your child's paperwork being cleared for the 1st day of practice/tryouts.

    All physicals must be within 365 days of March 2, 2018 which is first official days of Spring practice as per NJSIAA regulations.

    All the physical forms should be turned, the FamilyID.com Online Registration completed & the Activity Fee's satisfied at least 7 DAYS PRIOR to insure your child's paperwork being cleared for the 1st day of practice/tryouts.

    *A portion of the packet will be completed by you
     
    *A portion of the packet will be completed by a physician, if a new physical is needed
     
    • Date of Examination of the Sports Physical must be within 356 days of the 1st day of the athletic season.

     Step 3:  Submit Activity Fee Payment (if not done so already)
     
    (*FALL SPORTS note -- ACTIVITY FEE PAYMENTS CANNOT BE ACCEPTED UNTIL
    AUGUST 1ST OF THE UPCOMING SCHOOL YEAR)
     

    Option 1: Preferred method of payment is the electronic payment system that will be accessible on www.lrhsd.org/Page/601.
    On Left Margin click payment for Student Activities Fee to submit the fee for the school year. Cash payment will not be accepted.
     
    Option 2: Download payment form at www.lrhsd.org/Page/601 and submit form along with check, money order or certified bank check to the school athletic and activity office. Include the student ID# on the check. Make checks payable to Lenape Regional High School District (LRHSD).

     Step 4:  Submit Completed Physical to the Trainers' Office
     
    Submit the Completed Sports Physical/Medical Paperwork to Mr. Jeff Wood, Trainer in Cherokee’s South Main Office or the Trainers’ Room 1 WEEK PRIOR TO THE FIRST DAY OF PRACTICE/TRYOUTS.

    PLEASE NOTE: No Athlete will be allowed to participate/tryout until ALL of the above steps are completed by the deadlines mentioned above and ALL of their paperwork has been processed through the Athletic Department.

    Physical Examinations must be reviewed and approved by the Lenape District School Physicians prior to an athlete being declared medically eligible to practice. This process could take up to 3-5 days.  

    Athletes are encouraged to have all their paperwork turned into the Athletic Training Room as soon as possible prior to the start of the season. The Trainers strongly suggests the completed packet be submitted 1 full week prior to the start of the first day of practice/tryouts.

    The following rules and regulations that all attendee(s) are expected to adhere. Any attendee(s) who fails to comply with any portion may subject that attendee(s) to removal from the premises and forfeiture of attendance at future contests:

    a) NO possession or consumption of alcoholic beverages

    b) NO possession of or use of drugs

    c) NO smoking in any area of the school building or school grounds at any time

    d) NO radios

    e) NO posters, banners or noise makers of any kind

    f) NO re-admittance to paid events without ticket stub

    g) “Home” and “Visitor” seating sections will be posted and all spectators should remain in their respective stands

    i) Cheers are expected to be positive in nature
     
    h) Parents/Guardians/Spectators are to remain a safe distance from both the athletic fields, coaches and players
     
    j) NO ANIMAL (leashed or not) shall be allowed on the school campus for the possible safety, allergy and unhygienic threat they pose to the students, staff, athletes, coaches, volunteers, guests and spectators. The animal prohibited areas include but are not limited to the: school building, athletic venues, spectator areas, and parking lots. Unless the animal is a documented service animal accompanied by an individual with disabilities they must not be brought to the campus without the prior written permission of the supervising school administrator. This extends to all people who attend events at Cherokee High School.
